Follow Form 1 of the Schedules.

Required content

Include the following:

• grounds of appeal and a brief statement of reasons 
• applicant’s address 
• respondent’s address
• facts constituting the action
• specific issues for decision
• reliefs and orders sought
• request for information from the Procuring and Disposing Entity where necessary

Mandatory attachments and submission format

Attach the decision Include the specific decision being challenged.

Proof of payment

Attach valid proof of payment of the required fees.

Bound copies

Submit four (4) bound copies of the application in book form.

Cover page requirements

Include a title on the cover and a comprehensive index.

Page numbering

Each page must be consecutively numbered for ease of reference.
